What it actually does
Two logs, because they answer different questions
The audit log says what changed. The access log says who looked. In a business holding health information, reading a record you had no reason to read is the incident.
Queryable, not theoretical
This is a surface you can pull a report from, rather than a compliance claim in a footer that nobody can test.
On by default
It records because the system records, not because someone remembered to enable it on the right day.
