What it actually does
Signing means something
A note is signed by a named practitioner at a recorded time. That is the difference between a clinical record and a text box.
Corrections are additive
An amendment is recorded as an amendment rather than quietly replacing what was there. The original and the correction both survive, which is the entire point of a chart.

Every entry belongs to a client and a visit, so the history reads in order rather than as a folder of documents.
